This is a brief description of the "Five Why Analysis" technique used in root cause analysis. In simply terms you take a problem, ask why it occurred, then recursively ask why on every answer until it can't be asked any further or to a depth of five.
The results can then be documented in a Fishbone Diagram showing the various causes of a problem. It's quite a useful technique, and fairly easy to use.
