The Change Log template is used to record any changes to a server class computer. This template records the following information:

• The change identifier;
• The description of the change;
• The name person requesting the change and the date the request was made;
• The name of the person who approved the change and the date the approvalwas granted;
• The name of the person making the change and the date the change was made;

The template is straight forward to use. Simply enter the relevant details for the change as actions occur.
