This document template is intended to be used to document the requirements for an external contractor to provide one or more services to a project. It provides space for documenting the requirements, the acceptance criteria, and the reporting requirements that the vendor is required to support.

The template is divided up into a number of regions:

  • Header;
  • Requirements;
  • Acceptance criteria;
  • Reporting requirements;
  • Additional information;

Header Region

The header region contains administration information such as:

  • The request reference number;
  • The name of the person making the request;
  • The requestors contact phone number;
  • The date the services are required to be complete;

Requirements Region

The requirements region is where the services being requested are defined. It is important to ensure that each requirement:

  • Has an identifier;
  • Is unique;
  • Is unambiguous;
  • Is testable (where appropriate);
  • Is measurable (where appropriate);

Acceptanc Criteria Region

The acceptance criteria region defines how the requestor is going to accept that the requested services have been provided by the vendor. The acceptance criteria should address each requirement where appropriate. List multiple requirements in the same row if they have the same acceptance criteria. If the criteria apply to all requirements write "All" in the requirements field. The acceptance criteria should describe (where appropriate):

  • How each requirement will be verified and validated;
  • The quality expectations;
  • Appearance;
  • Performance levels;
  • Capacity;
  • Accuracy;
  • Availability;
  • Reliability;
  • Running costs;
  • Security;
  • Ease of use;

Reporting Requirements Region

The reporting requirements region defines how the vendor is to report progress to the requestor.The reporting requirements should address each requirement where appropriate. List multiple requirements in the same row if they have the same reporting requirements. If the reporting requirements apply to all requirements write "All" in the requirements field. For software development projects, the reporting requirements could include:

  • Feature list;
  • Feature burn down chart;
  • Unit test results;
  • FIT style acceptance test results;
  • Earned Value;

Additional Information Region

The additional information region is a freeform region where the requestor adds information supporting the request. Typically this will include:

  • Document referencs;
  • Statistical information erquired by the vendor;
  • etc;
