This is a template for a decision register. It is used to record and disseminate all project decisions within the team as well as outside. Use the decision register as an audit trail to show which decisions were made and by whom.

NOTE: Decision registers should be used in conjunction with a document management system to allow all changes to the document to be capture. This will assist with the audit trail to prevent unauthorised changes to the decision register from being made. The decision register captures the following information:

  • The date the decision was made;
  • The details related to the decistion;
  • The name of the decider;
  • A permanent or temporary decision. This is important because temporary decisions are those that are made to support a short term goal, but which need review at a later date to confirm that the decision is correct;
  • The review date for a temporary decision;

Using the template

Using the template is straight forward. It is intended to be used in a word processor such as OpenOffice.org Writer®™ or MS Word®™. To use this template, simply:

  1. Select the appropriate format of the decision register;
  2. Save the register to disc;
  3. Specify the project name in the file properties;
  4. Start recording decisions;
